If the HOA docs are ordered correctly, in CA, they use a form. The form is the questionnaire. However, despite what the contract states, most listing agents in my area have the title company order ... and they don't use the form. Garbage in. Garbage out.
The condo cert required by the buyers lender is most often paid for by the buyer. That, as Michael Jacobs states, is negotiable ... and is negotiated in the purchase agreement as to who pays for what.

HOAs here charge for the preparation of HOA documents for the buyer, oftens hundreds of dollars. The lender normally forwards a copy of the condo questionnaire - the Demand - to the HOA as well for completion. I don't think there is a separate fee, but for that buyer would pay

Yes, actually the seller has to have it filled out by another condo owner. Last year I sold a condo and the LA was taking his time getting these docs(it was holding up closing), there was another unit in the building on the market and I got them from that agent!

No it does not include the 921 or something like that. We provide for free the rules, decs, bylaws. Lender then might want that form & the mgmt company charges an arm & a leg. I've always fought that the buyer should pay.
Our contract does not call for seller to pay for lender questionaire. So it's an up in the air item.

I'm not familiar with a questionnaire required by lenders. That would likely be covered under the loan contingency, assuming the lender will not loan if it comes back with bad answers.
The rest of the docs are part of the buyer's inspection contingencies.
