I saw my first one a month or so ago when an agent on a team brought a buyer to one of my listings. It's primarily so the person running the team, John Doe of "The John Doe Team" for example, gets the credit for the transaction.
I don't work on a team, so that one example is all I have for you. Sorry. It's not a mandatory form as far as CAR is concerned, but you can bet your paycheck that your team lead will make it mandatory!
