I read the entire inspection report from cover to cover (acutally multiple times). I then I look for the systems which will keep you and your home dry (roof & walls & foundation). Everything else comes second.

I go through the summary page, and since not all inspectors are really good, I may question on something sepcific especially if it is out of their expertise area. Then I check with a licensed contractor in that field.

I don't have a " first thing ". The overall report is reviewed with buyer. Many times a buyer will say" i don't care about this or that, I would rather fix it myself.. or, I was going to rip that out and change it anyway, etc.

Jane - I always ask the inspector the overall condition of the home, and if they see anything that should be a concern to the buyer. Then I will review the summary information and ask the inspector anything I may have a question about, or that I don't understand. That way I can review the information with the buyer, and can be prepared in case the buyer has any questions.
