Those are things that sometimes come up.  Especially the utility bills an association fees, but I don't have a questionaire.

Questions that are required part of the disclosure for the real estate transaction Yes I will ask them. Personal usage of electricity NO, unless buyers wants to get an estimate. Right before the closing however I do ask the sellers if they have preferred landscaper or contractors they can recommend the new buyers.

Yes, I do... I want to have all these answers at my finger tips. I take down to who picks up trash, which days it's picked up, pet names, etc. It helps me get really familiar with the sellers and their home, but also when the selling agent has a question I usually have all the answers at my finger tips.

Unless someone asks for utility bills I do not ask the  sellers for it. Yes it is important to inform about association dues and what it covers.

Some of the information goes on the seller disclosure form they fill out. The other information I ask and write down or copy from seller records. I do collect all I can to help me sell it. I don't have a questionaire for them to fill out though.

Insurance, utilities, lawn service etc. are questions that arise often and although they'll vary owner to owner it's a piece to the puzzle that is frequently asked.

The association fees are usually on the print out of the listing I give to customer. Utility costs are addressed when the customer asks.  I don't think there's a need to put a specific number in writing, since the bills could dramatically be different depending on the number of people living in the home and their daily use of electricity. A list of phone numbers for cable service, electric company and water company is usually provided. I give the customer 2-3 names of insurance companies, if they don't already have one in mind.
